Your name has been randomly selected from the Voter’s Registration and Bureau of Motor Vehicles registration and driver’s license lists. We request that you complete the JUROR QUALIFICATION FORM and return it within ten (10) days. Please keep the SUMMONS portion and bring it with you when you appear.

GREETINGS: You are hereby summoned to appear before the Lake County Superior Court. Prior to your term of service, read this summons carefully and return the JUROR DISQUALIFICATION FORM within 10 ten days.

Directions to each court are on our website.  Jurors are paid $20 for attendance and $40 for each day in trial.  Mileage is .34/mile. You must immediately contact your employer about your service.  Please dress respectfully.  Average trials last 1-3 days.  If you serve as a juror; or serve until jury selection is completed, but are not chosen to serve as a juror; you will be excused for a period of 2 years.  If you need auxiliary aids or services under the Americans with Disabilities Act, please contact the court in writing.  Call 884-6100 for public transit information.

 YOUR  JUROR NUMBER has been assigned to identify you during your term of service.

YOUR JUROR NUMBER is located on the face of your summons along with your  TERM OF SERVICE and COURT ASSIGNMENT.

YOUR JUROR NUMBER is what you will listen for when you call our voicemail system or look online for your assignment. Normally, either a range of juror numbers will be called in or the message will report that no jurors are needed..

YOUR TERM OF SERVICE is one work week in length. You may be called to appear any day that week. You will call the voicemail or check the web page for information on when you are to appear on the Friday (or any time over the weekend) before your TERM OF SERVICE begins.

YOUR COURT ASSIGNMENT tells you where you are to appear. There are 4 possible assignments: CROWN POINT, HAMMOND, EAST CHICAGO AND GARY.

PLEASE FOLLOW THE INSTRUCTIONS FOUND ON THE VOICEMAIL FOR YOUR COURT ASSIGNMENT. 

COURT ADDRESS, PARKING, AND APPEARANCE TIMES ARE ALL LOCATED ON YOUR SUMMONS FORM.

 PLEASE REMEMBER TO BRING THIS FORM WITH YOU WHEN YOU APPEAR FOR SERVICE.
